What is the Hospital Discharge Program?
Our Hospital Discharge Program supports NDIS participants who are medically ready to leave the hospital but require suitable accommodation and supports to transition safely into the community.
We work closely with hospitals, the NDIS and other relevant stakeholders to identify appropriate housing options, coordinate essential supports and help make the transition from hospital to home as smooth and timely as possible.
