- Community participation
- Hospital discharge and accommodation-related enquiries
- BSP implementation support
Referral process
How referrals work
- 1
Send an enquiry
Send an enquiry with the participant’s consent, their preferred contact details and the supports they wish to explore.
- 2
We speak with the participant
Our team will speak with the participant and, where they choose, their supporters to explore preferences, available options and information needed to arrange supports.
- 3
Next steps
We will explain next steps and coordinate with the participant and the people they choose to involve.

Hospital discharge
Supporting a safe transition from hospital.
Our team works with hospital discharge planners, treating professionals and the NDIA to explore accommodation options and coordinate disability supports. Timing depends on each participant’s circumstances and the options available.
